• Mail completed rebate claim form and copy of sales receipt to Liphatech Resolv Rebate Offer, 3600 W. Elm Street, Milwaukee, WI 53209.

• Each purchase of one specially marked Resolv 16 lb. pail or one specially marked case of two Resolv 8 lb. pails qualifies for your choice of the offers.

• Limit of up to 5 rebates per person. Valid while supplies last.

• Multiple rebate forms can be sent in one envelope. Only one card needs to be completed when submitting multiple rebates, but all cards must be returned.

• Form and sales receipt from participating authorized Liphatech Distributor with eligible Resolv product circled must be postmarked no later than 9/30/12.

• Authorized Liphatech Distributors include official branch locations that distribute professional pest control products and are already setup in the Liphatech system as an authorized Distributor location. Drop ship locations do not qualify. Liphatech reserves the right to determine location eligibility.
